Would you like to add a Medical (Doctor consult) at the final appointment for a Fitness For Duty (non-workers compensation) medical certificate?

WESTRAC Pre-Approved Early Intervention Package Includes:

1 x initial session + 5 x subsequent sessions. Email or phone call for approval if further sessions may be required.
Short email following each session outlining: symptoms, work restriction advice, impression on diagnosis and when the next appointment is scheduled.
No formal reporting.
Escalation process - email or phone call to discuss if need for W/C claim to be opened up.
Consumables = maximum $200.00, approval required via phone or email for anything above this amount.

If multiple discipline services are required, costings will be as per each allied health service separately.
All costings are aligned with the respective state-based Workers Compensation gazetted rates.
Costs are inclusive of all approved consultations, consumables, communication and reports, in line with the state-based Workers Compensation gazetted rates.
Invoicing is completed following each session and will be itemised with consult, report, communication and consumables
